- Name (as per proof)
- Father’s name (as per proof)
- Active email address
- Personal mobile number
- PAN number (for exam fee payment via NTRP)
- Height in centimetres
- GPS latitude and longitude at your Aadhaar address (degrees, minutes, seconds)
Uploads (JPEG)
- Recent passport-size colour photo (plain background, decent dress)
- Scanned signature (blue or black ink)
Uploads (PDF)
- Date of birth proof (birth certificate or SSLC/HSc mark sheet showing DOB)
- Address proof: scanned Aadhaar (proper scanner — not mobile photo)
Portal-generated forms
- Nationality certificate (issued by A or B Grade Gazette Officer — notary advocate not accepted)
- Undertaking certificate for minors (ages 12–18)
- Exemption certificate for diploma/degree holders in electronics or telecommunications
- Proof of degree/diploma (for exemption applicants)
File sizes should be under 1 MB. JPEG for photos; PDF for certificates.
